What is a Geriatric Care Manager (a/k/a Aging Life Care Manager)?
A Geriatric Care Manager helps older adults and families make sense of the aging process. They are a guide through the maze of choices and questions, tailoring the plan to the client while navigating the complexities of the health care system and providing vetted local resources.
Role Description:
This is initially a part-time role, projected to transition to a full-time role as a Geriatric Care Manager/Aging Life Care Manager. You will be responsible for providing day-to day support and guidance to older adults and their families. This position requires you to travel to clients’ homes, retirement communities, assisted living or nursing homes, and to see clients in the hospital when needed. Interface with community and medical providers is required, both in person and via email, phone and other technologies. The clients we serve are predominantly older adults, although we do see clients with younger onset of disease processes.
The ideal candidate is a professional with a wide range of experience in elder care. This may include environments such as a client’s home, assisted living, retirement communities, hospitals, adult day health, VNA, hospice, home care, and skilled nursing facilities. Knowledge and experience working with individuals with Alzheimer’s and related disorders, and mental health challenges, is necessary. Comfort with medical terminology and knowledge of common diseases of aging is a requirement.
While this role is primarily based in the Boston/MetroWest area, travel to North and South Shore territories may be required.
Duties & Responsibilities:
· Conducting assessments and developing comprehensive care plans; coordinating and implementing care plans (as requested by clients)
· Identifying and coordinating community-based services
· Working in conjunction with clients and/or families, physicians and ancillary health providers to manage health and mental health care needs
· Communication and collaboration with providers, families, your team, the client and others, through telephone calls, emails, Zoom, and text messages
· Maintaining up to date and accurate records
· Accurate documentation of client information, care plans and daily care management work, including billing/EMR entry within 24 hours of the activity (same day preferable)
· Educating clients and families about options and resources
· Advocacy and caregiver coaching
· Proactively evaluating our clients’ goals and the steps needed to reach them
· Building relationships with community service providers and elder care professionals (attending marketing events as needed)
· Such other tasks and responsibilities as may be assigned from time to time
Qualifications:
· Compassionate, empathetic and competent, and being a good listener
· Experience in the workforce and in elder care (minimum 5 years, preferably 7+)
· MSW, LPN or RN required
· Experience in social services or a related field working in a clinical capacity
· Bachelor’s degree or higher in a relevant field
· Strong care management experience and elder care skills
· Flexibility and “out of the box” thinker
· Comfort with and managing challenging family dynamics
· Strong communication skills, excellent interpersonal skills
· Ability to create trusting relationships with clients and families
· Knowledge of community resources, entitlements, health insurance requirements and options
· Highly organized, detail-oriented, self-directed, ability to work in non-traditional professional work environments and manage fee-for-service caseload effectively
· Proficiency with Microsoft Word, Excel, Outlook, Zoom, mobile devices—laptop and smart phone, efax etc.
· Valid driver’s license and a reliable, insured vehicle
· Obtain and/or maintain applicable professional licensure as well as ALCA certifications
