Emily S. Grossman, MSW, CMC, CDP, is the founder of NextPath Care Management. With more than 23 years of experience in large and small healthcare organizations, she uses her industry knowledge and established relationships to develop comprehensive plans that get everyone — clients, families and healthcare providers — on the same page.
Emily’s extensive experience in all levels of senior care, including home and community-based services as well as institutional and insurance-based resources, has formed the foundation of her practice. Her attention to detail and proclivity for backup plans keep her clients, their spouses and families organized and well-informed so no healthcare needs and decisions are left to chance.
Early in her career as the Associate Director of Home Care for a growing home health agency in Greater Boston, Emily learned the ins and outs of senior home care. Through this experience, she gained extensive knowledge of home care services and what it really takes to make a successful plan of care.
Since then, Emily has lent her expertise as a consultant to the Massachusetts Executive Office of Elder Affairs (EOEA) for Case Management Training for an Aging Services Access Point (ASAP), as well as to multiple home care agencies for operational and case management training and review. Emily has also served as a consultant to the Combined Jewish Philanthropies Task Force on Jewish Aging in the Boston Area; as well as Continuing Care Retirement Communities and Visiting Nurse Associations in both the New York and Greater Boston areas.
Emily has delivered seminars on demystifying ‘the system,' housing and care options, and how best to plan for the future. Emily has also contributed to various newsletters and publications. including Aging Wisely…Wisdom of Our Elders, 1st Edition By Irving Silverman and Ellen Beth Siegel, in which she wrote the chapter: “We need to talk—when is it time to hang up the keys?”
Emily served as Treasurer of the Board for the New England Chapter of the Aging Life Care Association from 2015-2018 and currently serves on the Programming Committee. She has maintained membership in the Aging Life Care Association at the Advanced Professional level since 2011. Emily holds several professional credentials: Aging Life Care Professional® by the Aging Life Care Association®, Care Manager Certification from the National Academy of Certified Care Managers, and Certified Dementia Practitioner with the National Council of Certified Dementia Practitioners.
Emily earned a B.A. from Tufts University and a Master’s of Social Work with a clinical concentration in Older Adults and Families from Boston College, where she was a Litwak Fellow. She completed her graduate training at Hebrew Senior Life Continuing Care Retirement Community Newbridge on the Charles in their Long-Term Care setting and with Jewish Family Services of Metro West. Emily is an active member of the Tufts Alumni Association and is a Mentor in the Boston College School of Social Work Alumni Mentoring Program. When she’s not working, she can be found gardening, cooking, or out walking with her family’s dogs.
Cori Warzybok, MSW, LICSW, CDP has over a decade of experience working in a variety of healthcare settings, helping to manage complex care for clients and their families. She has considerable experience throughout senior care and health care, and has expertise in managing specialized medical care, Alzheimer’s Disease and dementia, mental health, chronic pain and substance use disorders. Cori knows how to thoughtfully facilitate interdisciplinary and family meetings to prioritize goals of care for her clients.
Cori’s wide-ranging professional experience from acute to post-acute and community-based care, has allowed her to fully understand the complexities of transitioning a patient through the entire care continuum. Most recently, Cori worked as a social worker at CHA Hospital in Everett, MA, in the Emergency Department, Med Surg floors, and ICU. While at Spaulding Hospital in Cambridge, MA, Cori was a case manager in oncology and gained extensive experience working with challenging end-of-life circumstances. Early in her career, Cori worked as a discharge planner on the short-term rehab (RSU) at Newbridge on the Charles, in Dedham, MA, where she worked with seniors and their families for successful transitions of care.
Previously, Cori worked as a Clinical Social Worker at Cambridge Health Alliance’s PACE (Program of All-Inclusive Care for the Elderly) program, where she led a monthly Memory Cafe for seniors experiencing early to mid-stage dementia. She worked as part of an interdisciplinary team to assess and create care plans for patients with complex psychosocial, behavioral, and medical needs while providing in-home case management, crisis intervention, supportive counseling, and community resource coordination within the PACE model of care.
Additionally, Cori has traveled across the United States as a Travel Social Worker serving inpatient units and emergency departments in Arkansas and South Carolina, and has worked as a Hospice Social Worker for VNA Care in Worcester, MA. As a Case Management Intern at HouseWorks in Woburn, MA, Cori coordinated a large scale Caregiver Support conference that included several notable speakers in the aging profession, and created an evidenced-based, sensory activity dementia training for caregivers.
Cori earned a B.A. from the University of Massachusetts Amherst and Master’s of Social Work with a macro concentration in Older Adults and Families from Boston College, where she was a Hartford Partnership Program for Aging Education Fellow. Outside of work, Cori likes to hike with her dog Ralphie, play pickleball, and see live music.
